We offer a free 15-minute consultation with our providers to answer your questions and assess if our services might be right for you. Many questions can be answered on our FAQ page. You can also text or call us at 971-403-0884 or e-mail us at info@synaptic.care.
Booking an appointment
Schedule an appointment below. Our system requires a card on file to book. You will not be charged for the appointment when booking. You will then get an email invite to Charm, our health record system.
If using insurance for your visit, your copay amount will be billed to the card on file at time of visit.
Note: A consultation is required before booking a new patient intake with Dr. Matthew Hicks and Dr. Olivia Hicks.
